Instructions:

  1. Choose the template you want to use by either viewing the sample pdf of the template or running your mouse over the file

  2. Click on the Name of the template you want to download

  3. Make sure you save the template as a .dot not a .doc and then Click Save
    (If you are unable to download the file as a .dot then click on the zip file and download that instead.
    If you have downloaded the zip file please extract it to the 'MS Word template' folder on your server)

  4. Save the file to the Medical folder in the 'MS Word template' folder on your server
